trust (such as an adult child)
to have access to your medical
information (needs to be
renewed annually);
• An Advance Directive or
Living Will allows you to
express your wishes for
medical decisions so that if
you are incapacitated, your
voice will still be heard;
• A Power of Attorney (POA)
agreement for financial and
medical decisions made
on your behalf by a trusted
individual that you appoint;
• A Last Will and Testament to
ensure that your final wishes
are known and carried out.
Be proactive and develop a
solid, realistic plan now to protect
your independence - it is worth
protecting!
