We’re Connecticut Community Care, Inc. (CCCI). Since 1980, we’ve helped thousands of individuals live safely and independently at home. A private, nonprofit organization, we’re Connecticut’s largest care management service for the elderly and adults with disabilities.
Our nurse and social services care managers look at our clients’ full range of needs, from caregiving to housekeeping, from medical to financial. We find the right resources and coordinate the program. We provide services to clients on a fee-for-service basis through our private division, Care Management Associates, and to individuals who qualify for the state-funded Connecticut Home Care Program for Elders.
