Touchpoint: Healthy Life Transitions

Health Care Services in IndianapolisCommunity Health Network is taking an innovative approach to health care for patients age 55 and older. Touchpoint will bring together clinical and retail health care resources to offer healthy life transitions at all levels of care.

"We named the program Touchpoint because we are providing answers," says Jessie Westlund, R.N., CEO of Community Home Health Services and director of Life's Journey and Touchpoint. "We want to reach out to people age 55 and older and point them in the right direction when they have questions."

Retail services include an online membership tied into the myCommunity program, where Touchpoint members will have access to a variety of services as well as social opportunities, clubs and future travel opportunities. At Wellspring Pharmacy, Touchpoint members will have special discounts through the pharmacy's Silver Club, and Pro Health will be offering or making available Medicare Advantage products. Touchpoint alsos encourage members to engage in their health through special programming including classes and lunch and learns, and with regular newsletters and wellness topics.
