At Gateway HealthSM, we understand that different people have different healthcare needs. That’s why we offer three distinct Medicare Advantage plans to Ohio residents.

Medicare Advantage Plans with Prescription Drug Coverage (MAPD)
These plans are available to Medicare beneficiaries who have Medicare (Parts A and B) and live in the Medicare Assured service area. Our Medicare Advantage plans provide high quality, all-inclusive health coverage packed with value AND prescription drug coverage. It’s a better way to manage your health and get the care you need so you can stay healthy and fit.

Medicare beneficiaries must enroll in one of these MAPD plans during the Medicare Annual Enrollment Period in the fall of each year, unless they qualify for a Special Election Period.

Chronic Condition Special Needs Plans (CSNP)
These plans are available to Medicare beneficiaries living with diabetes, cardiovascular disorder or chronic heart failure. Our Chronic Condition Special Needs Plans offer comprehensive health coverage geared at taking care of your condition and reducing the risk of developing serious health complications.

There are no income requirements for CSNP plans, but potential members must have
Medicare and one of the qualifying chronic conditions, as well as live in the Medicare Assured service area. Year-round enrollment is available.

Dual Eligible Special Needs Plans (DSNP)
These plans are available to Medicare beneficiaries who also qualify for Medicaid or receive assistance from the State. Our Dual Special Needs Plans deliver primary and specialty medical care, hospitalization and prescription drug coverage, with extra benefits that help you maintain optimal health.

Year-round enrollment is available to individuals who meet the income requirements and live in the Medicare Assured service area.

Gateway HealthSM offers HMO plans with a Medicare Contract. Some Gateway Health plans have a contract with Medicaid in the states where they are offered.  Enrollment in these plans depends on contract renewal. Gateway Health Special Needs Plans are available to anyone with Medicare and Medicaid, or Medicare and diabetes or cardiovascular disorder or chronic heart failure.
