Living Independently For Elders
LIFE (Living Independently For Elders) offers comprehensive care to frail elderly individuals in West and Southwest Philadelphia who are nursing home certifiable, but prefer to remain in the community.
LIFE is a managed care program, combining Medicare and Medicaid benefits to cover all levels of care and all health care needs of our members: primary, preventive, acute, and long-term care. LIFE is a national PACE Program (www.npaonline.org).

LIFE services are provided by an interdisciplinary team of RNs, LPNs, CNAs, and other health professionals including physicians, nurse practitioners, PTs/OTs and social workers, and include:
- Adult day care
- Acute/sub-acute inpt.
- Transportation
- In-home services
- Personal care
- PT/OT
- Primary care
- Medical specialty services
- Pastoral care
- Medications
- Restorative support services
- Social services
- Nursing care
- Nursing home
- Emergency care
- Meal Planning (special diets)
- Home-delivered Meals
Members must receive all services through LIFE's network or they may be liable for paying for those out of network services. LIFE is owned and operated by the University of Pennsylvania School of Nursing and operates out of two sites to cover the West/SW Philadelphia catchment area.
