SAGE Eldercare
SAGE Eldercare is a nonprofit community agency with over 54 years of experience assisting older adults, their families and caregivers. Our range of services help the elderly remain independent in their homes for as long as possible. Since SAGE Eldercare opened its doors in 1954, this organization has distinguished itself as not only as one of the oldest community-based eldercare providers in New Jersey but also as one of a few of its kind in the United States to offer a comprehensive range of services. SAGE Eldercare is a pioneer in providing community-based care for the elderly, starting one of New Jersey’s first Meals-on-Wheels programs in 1966 and adult day care programs in 1975. Many of the services and programs offered by SAGE Eldercare today have been used as models by other public and private agencies for their programs.
SAGE Eldercare’s mission is to "support the independence, well-being and quality of life of older adults, their families and caregivers, through the provision of client-centered health, social and support services."
SAGE Eldercare is committed to high quality, innovative programs and services. Beginning with eight home health aides in a local hospital, SAGE Eldercare has grown to offer 12 programs staffed by 20 full-time professionals. 28 part-time support staff, 72 Home Health Aide Caregivers and more than 400 volunteers. Today, the primary programs at SAGE Eldercare include Spend-A-Day Adult Day Health Center, InfoCare: Referrals and Information, HomeCare, Meals on Wheels, Bill Paying Service, Small Home Repair Service, SHIP (State Health Insurance Assistance Program of Union County), Grocery Shopping & Errand Service, Fall Prevention Program, the Eldercare Planning and Guidance Program, Caregiver Support Groups, and Educational Programs. In February 2005, SAGE proudly moved into its new, state-of-the-art facility located at 290 Broad Street in Summit, New Jersey.
SAGE is licensed by the New Jersey Division of Consumer Affairs; Accredited by the Commission on Accreditation for Home Care; Licensed by the New Jersey Department of Health and Senior Services; Charter Member of the New Jersey Adult Day Services Association; Member of the New Jersey Chapter of the Meals on Wheels Association of America; Member of the Meals on Wheels Association of America; Member of the Alzheimer’s Foundation of America; and a United Way Member Agency.
